Duplicate Certificate or/and Pass Book :

I. Read following General Procedure before filling application form :
1.Download application
2.Use BLUE colour pen

3.Write in Capital letters
4.Write name EXACTLY as in Diploma/Degree Certificate
5.Write full name of the college
6.Write the serial number of the college as per A.P.P.C website Click here
7.Write address as in ‘address proof document’ submitted by you
8.Use recent colour photos
9.Submit Demand Draft for verification of degree by the respective University, if required
10.Tag photocopies of documents to the application as per CHECKLIST
11.Submit application in person
12.Get Original Certificates stamped and endorsed to complete registration process

II. Application Checklist :
1. Application
2.Police report in Original
3.Notarized Affidavit-B
4.Employment proof ( Drug licenses/Teaching/Industry etc ID proof )
5.Residential proof(Election ID/Aadhar card/Bank account/Indian Passport/ration card/driving license)
6.Photocopy of RP Certificate
7.Passport size 4 colour photos
8.Fees (Cash/DD)
9.Collect Duplicate Pass Book on same day

III. Fill the application and tag all the documents in sequence as per CHECKLIST
IV. Submit application IN PERSON in the office of Andhra Pradesh Pharmacy Council.
V. Get Originals back after verification.

About Us :
Andhra Pradesh Pharmacy Council is a statutory body constituted by the Government of Andhra Pradesh u/s 19 of The Pharmacy Act of 1948 consisting of 6 members elected by Registered Pharmacists among  themselves, five members nominated by Government of Andhra Pradesh, one member elected by AP Medical Council and three Ex-Official Members.

Presently Director General, Drugs Control of AP is in-charge of the Council. Registrar is on full time from 23 May 2013.

1. Can I get Good Standing Certificate/Verification certificate by hand.
No. The Certificate will be sent directly to the country by A.P.P.C.

2. If prospectus of other country demand can I get certified copies/attestation of my R.P.Certificate.
Yes
3. What is the period of Renewal
Every 5 years

4. What is the time span for getting Duplicate Certificate & Pass-Book
A) Duplicate Certificate : Within 3 days after receiving information from all the Drug Licensing Authorities in AP.
B) Pass Book : On the same day of submission

5. What is the time span for getting Registration
Diploma : 15 Days for AP State holders. 15 days after receiving confirmation from state Boards for outside AP state
Degree : 15 days after receiving confirmation from University.

6. What is the time span for getting Good Standing/Others Certificate
4 days
7. What is the time span for getting Re-Registration Certificate (by Transfer from Other States)
Fifteen days after receiving No-Objection Certificate from other State Pharmacy Councils.

8. Is Registration needed for foreign studies
For foreign employment or writing equivalent examinations they are asking APPC Registration. Other countries are also demanding Good Standing Certificate from the Council to consider for employment/permitting for examination
