Keeping the paradigm shift in approach of Government to health care, the State has an ambition to increase affordable and quality health coverage to all the citizens.

In order to reduce out of pocket expenditure on health and to give better health coverage to the common man, the Health Department Haryana has now launched a new all comprehensive and path breaking scheme called Mukhyamantri Muft Ilaaj Yojana which seeks to provide free treatment to all the citizens and covering all essential components of curative health care.

Under the Mukhyamantri Muft Ilaaj Yojana, the following services shall be provided completely free :
** Free surgeries upto secondary level and subsidized at tertiary level under Surgery Package Programme to all residents of Haryana in all the Government Health Institutions including Medical Colleges.

** Free supply of essential medicines to all patients in OPD/IPD/Emergency.
** All Indoor Services.
** Free investigations/diagnostic services including X-Ray, Ultrasound and ECG.

** Free referral transport/ambulance services to all in emergency and in non-emergency condition free transport of all pregnant women for delivery drop-back and post delivery check up and sick children upto 5 years.

** Provision of free Dental treatment to all the residents of State coming in Govt. Health Institutions as well as PGIDS , Rohtak.

Documents which shall be accepted as proof of residence :
1. Passport
2. AADHAR Card
3. Ration Card
4. Driving License
5. Voter ID Card

6. Government Photo ID cards/ service photo identity card issued by PSU
7. Electricity Bill (not older than 3 months)
8. Water bill (not older than 3 months)
9. Telephone Landline Bill (not older than 3 months)
10. Property Tax Receipt (not older than 3 months)

11. Bank Statement/ Passbook
12. Post Office Account Statement/Passbook
13. Haryana Domicile Certificate
14. MNREGA Job Card
15. Freedom Fighter Card

16. Kissan Passbook
17. CGHS/ECHS Card
18. Disability ID Card/handicapped medical certificate issued by the respective State/UT Governments/Administrations

19. Marriage Registration Certificate
20. Certificate From Employer Of Reputed Companies On Letter Head
21. Gas connection Bill

In case of spouse, the residence proof of the other spouse shall be valid but with any Government document showing the relationship with each other.

In case of minor, , the residence proof of the either of the parent shall be valid, but it should accompany with the school principal certificate of relationship with the minor or any government document showing the relationship.

About Us :
Health Department, Haryana is headed by the Director General, Health Services, Haryana and is assisted by One Additional Director General Health Services, 5 Director Health Services, One Additional Director (Administration), One Joint Director (Administration), at the Head Office.

There are 56 Hospitals, 114 Community Health Centres and 485 Primary Health Centres in the State. The head of health services at the district is Civil Surgeon while the civil hospitals are headed by Principal Medical Officers/Medical Superintendents / Senior Medical Officers.

Community Health Centres and Primary Health Centres are headed by Senior Medical Officers and Medical Officer in-charge respectively.
