New Copy of Certificate :
In case your registration certificate is spoilt or mutilated you may obtain a new copy of your registration certificate by fulfilling the requisite procedure at MSPC office

Procedure for new copy of Registration certificate:
1. Surrender original Registration Certificate.(In torn ,mutilated ,whatever condition)
2. Application for new copy.(given below)
3. Four 3.5X 4.5 cm size, identical, front pose ,recent photographs.
4. Identity slip duly attested by gazetted officer / Principal of school or college(given below)
5. Address slip duly filled in.
6. Remit Rs . 200/- (Provided registration is renewed up to date)
7. Proof of date of birth
Duplicate Certificate :
In case of loss of your registration certificate you can obtain a duplicate copy of registration certificate by fulfilling the requisite procedure at MSPC office in person
Procedure for Duplicate copy of Registration certificate (Loss of certifciate):
1. Submit original affidavit on Rs 100 /- stamp paper ,duly notarized (format given below) IN PERSON
2. Copy of police complaint regarding loss of certificate
3. Application for duplicate copy.(given below)
4. Four 3.5X 4.5 cm size, identical, front pose ,recent photographs.
5. Identity slip duly attested by gazetted officer / Principal of school or college(given below)
6. Remit Rs . 200/- (Provided registration is renewed up to date)
7. Proof of date of birth

How to notify change of name or address?
Write a letter addressed to the Registrar quoting your registration number and complete postal address along with pincode signed by you. You will receive a letter confirming your changed address.
How to obtain Pharmacist’s Professional Profile card?
To obtain a Pharmacist’s Professional Profile Card, download the following application form and submit the application form in person at MSPC office along with the following documents.
** Cash /D.D. of Rs.200/-towards PPP charges
** Photo copy of Registration Certificate (front back). (Please bring your original Registration Certificate for verification)
** Two front pose recent photographs size 3.5×4.5 cms.
** Proof of date of birth (if not recorded In Registration Certificate.)
** Photo copy of recent renewal receipt.
How to renew Pharmacist’s Professional Profile card?
To renew a Pharmacist’s Professional Profile Card, download the following application form and submit the application form in person at MSPC office along with the following documents.
** Cash /D.D. of Rs.100/-towards PPP charges
** Photo copy of Registration Certificate (front back). (Please bring your original Registration Certificate for verification)
