Step- 5:
Address:
Select the Option for “Is applicant’s present address different from that in the existing passport?”

Option-1: Yes

Step-6:
After Selecting, Submit the “Continue” button.

Step-7 :
Reason(s) for Re-issue :

Step-8:
Submit the “View Required Documents” button to get the list of documents for reissue.

2.Validity Due to Expire
Within one year before expiry of old passport

3.Validity Expired
** Within three years after expiry of old passport
** After three years of expiry of old passport

4.Renewal of Short Validity Passport (SVP)

If Damaged, specify extent of damage:

Note:
** Applicants are advised to visit their jurisdictional passport office’s home page under Passport Office Page to know about additional documents required, if any.
** Applicants are required to furnish original documents along with one set of self-attested photocopies of the same at the Passport Seva Kendra (PSK) for processing.

Minor:
** It is assumed that consent of both parents is available, unless specified.
** For minor applicants, present address proof document in the name of parent(s) can be submitted.
** It is advised to carry original and self-attested copies of parents passport to Passport Seva Kendra (PSK), in case parents possess passport.
** For minor applicants, documents can be attested by parents.

Adult:
** Including Senior Citizens
