Check List of Documents to Applying for New Licence through Online License Management System:
1. Challan for Rs 1000/- towards fees2. Seed license issued by concerned state dept of Agriculture (translated copy if it is drafted in languages other than English and Hindi) in case of seed companies established outside AP seeking marketing permission in AP

3. Bio-data of person responsible for legal correspondence (on a non judicial stamp paper of not less than Rs 100/- with notary) bearing the responsibility of the compliance to terms & conditions of the license. (With residential address and phone no & Photo)

4. Nomination of authorized signatory(or)person responsible for day-to-day administration along with photo (on a non judicial stamp paper of not less than Rs 100 with notary)

5. Agreement with C&F Agent appointed, in case of seed companies established outside AP seeking marketing permission in A P
6. Other documents, if any

7. Declaration duly indicating that the proprietor not convicted under Seed laws,1966 or Essential Commodities Act,1955 or Environmental Protection Act,1986 for preceding 3 years
8. Copy of Self declaration

9. Declaration duly indicating that the partners not convicted under Seed laws,1966 or Essential Commodities Act,1955 or Environmental Protection Act,1986 for preceding 3 years

10. Copy of partnership deed
11. Declaration duly indicating that the Head of company itself are not convicted under Seed laws,1966 or Essential Commodities Act,1955 or Environmental Protection Act,1986 for preceding 3 years
12. Copy of memorandum of articles of association
13. Declaration duly indicating that the Head of the Institution itself are not convicted under Seed laws,1966 or Essential Commodities Act,1955 or Environmental Protection Act,1986 for preceding 3 years

Required Documents For Licence Application

Following documents are required for Application For The Grant Of Licence To Manufacture Insecticides:

Enclosure Name:
1 Fee Particulars(original Challan should be enclosed)
2 Status Of The Firm(whether proprietary/Partnership/ Private /Public limited company proof to be enclosed)
3 Name of the Person Responsible For day-to-day administration, age, father’s name/husband name, designation, resdential and official address.Consent on N.J.S.(Non judicial stamp) (Affidavit), contact number
4 Name of the Person Responsible for Quality Control (Expert Staff), age, father’s name / husband’s name, designation, residential and official address with contact number
5 Bio-data of the Expert Staff(Chenist) attested by concerned A.O
6 Expert staff(Chemist) Degree Certificate
7 Affidavit of Expert Staff (Chemist) on NJS on worth of Rs.20/-
8 Undertaking of the M.D on N.J.S woth of Rs.20/- as per the Section 33 of insecticides Act 1986.
9 Acceptance letter of Medical Doctor.
10 Certificate of Industries department / NOC fron Gram panchayat or Municipality.
11 Copy of resolution of the Board of Directors of Partners as the case may be in case of limited company or partnership authorozing a person to represent on behalf of the firm with complete address
12 Consent letter from A.P.P.C.B.(Not applicable for Bio-pesticides)

Note:
Copies of documents should be attested.

Fee Per Product :Rs.2,000/- and maximum Rs.20,000/- (i.e., 10 products and above).

Head Of Account:
Manor Head – 0401- Crop Husbandry.
Minor Head – 107 Receipts under Plan Protection Services.
