How To Apply For SHC Scheme?

Government of Uttar Pradesh has started Cashless Treatment Service to its employees. All Employee and Pensioners along with their dependents will be beneficiaries of this service.

Now every Employee/Pensioners will be issued a State Health Card(SHC) by State Health Card Cell managed by Directorate of Medical Health Uttar Pradesh.

Therefore after getting Health Card every cardholder will become Eligible to get Treatment at any Government or Recognised Hospital free of cost.

At the initial stage this facility has been statrted at all government hospitals. Government of Uttar Pradesh is in the process of recognizing number of private hospitals under this scheme.Hence this service will be extended to private hospitals soon.

SECTS Employee Gateway Gives following facilities to state employee :
1. Direct Apply For State Health Card.
2. Download/Print Filled Application.
3. Check Application Status.

Note :
State Employees who are Directly Applying for State Health Card will have to Download Filled Application and Submit Signed Copy to there DDO along with all necessary Document to forward there Application to SECTS Cell, UP. Applications Forwarded by DDO will be Considered for the Issue of State Healt Card (SHC).

To apply for State Health Card please enter the following details,
1. Select Department Name
2. Select Office District
3. Select DDO code or Treasury Code (DDO code for Serving Employees & Treasury Code for Pensioners)

4. Select Office Name
5. Enter Applicant’s Name
6. Select Date of Birth
7. Select Card Type (Serving or Pentioner)

8. Select Present or Last Post
9. Enter Present or Last Basic Pay
10. Enter Mobile Number
11. Enter E-Mail Id

12. Enter Aadhar No
13. Enter Address for Correspondence
14. Upload Picture (Photo size < 20KB, 170 x 137 px & jpg/jpeg format)
