How To Change of Name in Birth Certificate?
Service Description :
Applicant can fill an application e-form application for change of name in birth certificate.

How to apply :
The applicant who wants to apply for change of name in birth certificate shall submit the e-form duly filled-in. He/she shall state true facts in the application. Follow below instructions to access the application,
** Click on Services and Forms on the portal.
** From the department list, select Local Administration.
** List of all services will be displayed. Depending on the method of submission, click one of the links under Action.
** Fill the online form, attach supporting documents and submit.
** Acknowledgment with unique registration number will be displayed.
** Using the unique registration number, the user can check the status of the application on the state portal.
** Once the status is complete the user has to meet the concerned department person along with the required original documents and collect the certificate from the department person.
