Form “VI” of insecticides rule 1971
Documents To Be Enclosed :
** List of products.
** Principle Certificte issued by the dealer.
** Two recent passport sixe photographs.
** Proof of Identity.
** Description of place for where the sale depot intended to operated.
** No objection certificate of the tenant if the business is intended to be carried out on the land of tenant other than the applicant.
** Recent copy of Form-F of the land where business is intended to be operated.
