Documents Required For PMC Urban Poor Medical Assistance Scheme

1) Municipal Corporation Pune Slum resident ID card or 2020 onwards or current year receipt/service charge receipt or complete below poverty line ration card and in addition to the above the family residing in Pune M.N.P area has an annual income of Rs. Up to one lakh Hon. Certificate from Tehsildar, Pune
2) Ration card
3) Birth certificates of children
4) Two photographs of eligible family members (visiting card size)

Execution Period:
The scheme will be implemented every year for a period of one year from 1st April to 31st March.

5 Annual Subscriptions :
The registration fee for the family is Rs.100 / – per year and Rs.100 / – per annum. Rs.200 / – Cash charges will be levied.

What is PMC Urban Poor Medical Assistance Scheme?

Urban Poor Medical Assistance Scheme is a Pune Municipal Corporation. Living in working area slums, Yellow ration card below the poverty line and annual family income Rs. Rs. This scheme is applicable for the period from 1st April to 31st March every year.In order to become a member of this scheme, the identity card of the corporation living in a slum or G.V.N.
