How To Apply For Change of Name Certificate in TN Dental Council?
** The applicant should come in person to the office.
** All particulars of the application must be filled by the applicant.
** The name and particulars entered must be exactly correspond with the name and particulars of the applicant entered at the University.
** Documents to be submitted along with the Change of Registration Certificate Application Form
** Directly scan the original certificates in color and upload them in the online application.
** Documents to be submitted along with the Change of Name Certificate Application Form
** All Original Certificates & Address proof take (A4 Size) Colour Xerox copy only. It’s Mandatory.
** Take a (A4 size) colour printout of the application form in a single side (Don’t take front and back).
** Original Registration Certificate (issued by the Tamil Nadu Dental Council) has to be surrendered herewith for Cancellation.
** Change of Name Certificate (issued by the University only accepted).
** Two recent color passport size photos – Same photo uploaded in the online should be submitted at the time of Registration.
** Aadhar Card is a must. (If Aadhar is not available, optionally you can submit Voter id or Valid Indian Passport are only accepted). Any one of the proof with Front & Backside taken on a single page – Color (A4 size) Xerox must be submitted.
** If a Candidate cannot come in person – he / she will submit his / her Form duly signed and give a separate authorization letter and mention his / her Name & Photo ID proof (A4 size) Color Xerox to someone.

If I am a Foreigner, can I get a Good Standing Certificate?
As per The Dentists Act 1948, no person other than Citizen of India shall not be entitled for registration by virtue of a qualification. Hence we cannot register a Foreigner and without registration we cannot issue Good standing certificate to them.
How can I get the NOC?
By surrendering the Original Tamil Nadu Dental Council Registration Certificate along with a application form stating that which State you require NOC, by personally or through any authorized representative. You have to pay Rs 500/- through Online, if your renewal is up-to-date.
Can I Practice anywhere in India by Registering with Tamil Nadu Dental Council?
Yes. As per Section 46(5) of The Dentists Act 1948, you can practice anywhere in India by registering with Tamil Nadu Dental Council.
How can I get duplicate certificate if the original certificate lost?
By lodging complaint with concerned police authorities, the original FIR along with an Affidavit from Notary Public should be produced for Duplicate Registration Certificate. It will be issued on payment of prescribed charges.
Can I get refund for online payment, whether a certificate or services cannot be awarded?
No, there is no refund if any certificate / services are cancelled due to technical difficulties, procedural difficulties and any other administrative reasons.
Can I send request for certificates and any other services from any one E-mail Id?
No, the candidate can apply certificates and all other services only through registered E-mail Id.
