** Candidate(s) need to submit the scanned copies of all the mentioned testimonials along with the online payment receipt to the council’s email: info@wbdc.org.in
** Then council will verify the aforesaid documents.
** Council will inform the candidate(s) over phone/email for attending physically himself/herself at the council office once.
** Candidate(s) need to produce all the above-mentioned originaltestimonials along with the online payment receipt at the time of visiting council office.
** Council will provide the new registration certificate to the candidate at that time.

Note:
Candidate(s) are hereby informed that they are required to visit the council office only once for physical verification of testimonials required for this purpose and collection of the registration certificate.

Required Documents For BDS Registration

The following documents are required to be arranged strictly)
1. Application Form for BDS Registration duly filled by the candidate
2. Four years BDS Mark-Sheet
3. Internship Completion Certificate
4. Copy of Provisional Registration Certificate
5. Copy of BDS Degree Certificate issued by the University
6. Class 10th Admit / DOB Certificate
7. Class 12th Passed Mark-sheet
8. Address Proof (Aadhar Card/ Voter ID/ Passport/ Driving License/ Electric Bill/ Bank Pass Book)
9. 2(two) copies colour Passport Size Photo(This will be required at the time of visiting Council Office)

Form of Declaration:
( See regulation 3 of the Dentists (Code of Ethics) Regulations 1976 )
(i) I solemnly pledge myself to devote my life to the cause of serving humanity in the field on dental care;
(ii) I shall not use my dental knowledge contrary to the laws of humanity;
(iii) I shall not permit consideration of religion, nationality, race, caste and creed, party politics or social standing to intervene in my duty towards my patient and the profession;
(iv) I shall look after the dental health of my patient as my first consideration;
(v) I shall honour the secrets which are confided in me by my patients during the professional services:
(vi) I shall always maintain the honour and noble traditions of the dental profession;
(vii) I shall deem it an honour to cherish a proper pride in my colleagues and shall not disparage them by me action, deeds or words:
(viii) I shall abide by the various provisions of the Act and desit from using a degree/diploma or an abbreviation indicating or implying a dental qualification, which I not in accordance with the definition of recognized dental qualification as defined under clause (j) of section 2 of the Act;
(ix) I shall not indulge in any activity which might bring discredit to the dental profession.
