Getting started :
When you register at any Apollo hospital, an Apollo Prism account is automatically created for you. You then only have to verify and activate this account online by clicking on the Sign Up button on the home page.

Sign In To Access Your Health Records :

Registration

1. Go to the above link, New User click on “Sign Up” button,enter the following details to fill registration form.

2. Enter the Mobile Number Click on Next button.

Activation :
** If your UHID and mobile number match, confirming an authentic user, we will send you an activation code to your mobile number.
** Next, enter the activation code received on your mobile
** Fill up the rest of the form with the requisite information.
** Once your form is complete, your account is activated and you are ready to start using Apollo Prism.

3. Please select the account you want to create ,then click on Activate button.

Your information on Apollo Prism :
Based on your encounter at the hospital, your Apollo Prism account will show any test results, discharge summaries and health check reports from the hospital. While most lab test results will appear in your account, certain test results may not, as they are deemed sensitive by the hospital.You will be able to see your health information on Apollo Prism within 24 hours, after report creation.

Error / Problem:
1. I have lost my UHID number. So, how can I get this and I tried several time to sign up into Apollo Prism but, after putting my mobile number I can’t get the code. Is there any alternative to get this code?

2. I had my medical check-up at your hospital in February. I received a message in my mobile that some of my lab recent reports are updated and asked me to check online. Please, I am unable to open the website.

3. What to do if the lab results are not uploaded to my PRISM account? Which number to call to get it uploaded? Will they send my reports to my mail if requested due to residing at far place?
