Ration Card Electoral Roll LIC Policy Passport Pay slip issued – Govt. Office (or) a Local Body
School Certificate Birth Certificate Affidavit Sworn by the applicant before an Executive (or) a First Class Judicial Magistrate (or) Notary Public.
Registration Certificate Insurance Certificate (If the vehicle is Registered one)
** amount of Fees payable Permit fees Rs.300/- + Endorsement Fee Rs.25/- + Service Charge Rs.50/- for each Region = Total Fees Rs.375/-
** should one apply Regional Transport Officer or Motor Vehicle Inspector Grade-I/II
** Time limit for service to be rendered 5 Days

Motor Cab (Taxi) Permit
** apply for Motor cab permit Owner of the vehicle
** the documents to be given Application in Form-CCPA
