How to Apply for Disability Certificate in Delhi ?

Issuance of Disability Certificates :
Existing Procedure
The Department of Health & Family Welfare, Government of NCT of Delhi has been issuing Disability Certificates to the Persons with Disabilities from last many years. The Disability Certificates are issued by the Notified Government Hospitals. As on date the notified Government Hospitals are follows

For Mentally challenged:
1. IHBAS:
The total number of Disability Certificates in Delhi issued up to 31.01.2010 is 23,413 (Source – Office of the Chief Commissioner Disability).

The procedure for issuing Disability Certificates to the physically and mentally challenged persons by all the notified Hospitals under the Government of NCT of Delhi is as follows:-
The following documents would be required to be submitted by the Handicapped Person (applicant) in the Office of the Chairman of the Physically Handicapped Board (i.e. the MS) of the concerned Hospital:-
** 4 Passport Size Photographs
** 2 Full Size Photographs
** Attested copy of the Ration Card of the Applicant
** On having done so, the applicant would be given a date for appearing before the Medical Board within 21 days of his furnishing the documents as in Para-1 above.

An OPD Card will be prepared and issued to the applicant from the Board Office with the help of which the applicant would be required to attend the OPD of the M.O. concerned and get himself/herself examined. Necessary investigations would be carried out by M.O concerned & necessary entries made in the Medical Card, within a week of the applicant presenting himself in the OPD.

Applicant would need to present himself before the Board on the date fixed for such appointment as in Para-2 above.

The Handicapped Person will be examined by the designated Board of Doctors, along with report received as in Para-3 above.

On the recommendations of the Board, a Physically Handicapped Certificate would be issued to the applicant in the prescribed format (on which a Passport Size Photograph of applicant would be affixed).

The Appellate Authority for any disputes with regards to the issue of Disability Certificates is the Medical Superintendent of the concerned notified Hospitals

Note:
If a designated doctor of concerned specialty who has been authorized to issue disability certificate, is on leave or is not present, a suitable alternative arrangement will be made by the Medical Superintendent of the concerned hospital, who will ensure the issuance of disability certificate within the time period specified in rule 4(2) of the Persons with Disabilities Rules 1996, as amended.
