Requirements of MPMC Registration

REQUIREMENTS FOR PROVISIONAL REGISTRATION :
The Applicant is required to submit the following documents in original with its photocopies:
** High School / H.S.S. mark sheet / Certificate in support of Date of birth
** Final M.B.B.S. mark sheet of Part I & II.
** Internship under doing certificate in original from the Dean of concerned Medical College
** 3 Rrecent passport size Black & White photograph (not more than of 5cm x 4 –1/2 cm)
** For registration fees & penalty amount, please see the prescribed fees chart.
** The Registered Medical Practitioner of Maharashtra Medical Council is required to attach a letter of N.O.C. of Maharashtra Medical Council.
** The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.

** Internship Completion Certificate from the Dean / Principal of the Medical College.
** M.B.B.S. Degree / Provisional Degree.
** 3 recent passport size Black & White photograph (not more than of 5cm x 4 –1/2 cm)

** The Registered Medical Practitioner of Maharashtra Medical Council is required to submit the letter of N.O.C. from the Registrar, Maharashtra Medical Council, Mumbai.

REQUIREMENTS FOR ADDITIONAL REGISTRATION OF THE P.G. MEDICAL QUALIFICATION :
The Applicant is required to submit the following documents in original with its 2 photocopies:
** Original Permanent Registration Certificate of MBBS of the M.P. Medical Council.
** Original recognized Post Graduate Degree / Diploma.
** For registration fees & penalty amount, please see the prescribed fees chart.
** The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.
** Name of your Medical College with a proof of authentic document.

REQUIREMENTS FOR OBTAINING DUPLICATE REGISTRATION :
The Applicant is required to submit the following documents in original with its 2 photocopies:
** Photocopy of Registration Certificate
** High School / Higher Secondary mark sheet in support of Date of birth,
** Final M.B.B.S. mark sheet of Part I& II,
** Internship Completion Certificate from the Dean / Principal of the Medical College.
** M.B.B.S. Degree / Provisional Degree.
** F.I.R. (for lost / Theft / missing / accident)
** An Affidavit in the enclosed Proforma –P
** 3 recent passport size Black & White photograph (not more than of 5cm x 4 –1/2 cm)
** The prescribed fee (see fee chart).

REQUIREMENTS FOR OBTAINING GOOD STANDING CERTIFICATE :
The Applicant is required to submit the following documents in original with its 2 attested photocopies:
** An Application on plain paper addressing to the Registrar, M. P. Medical Council.
** The prescribed application form in duplicate duly filled in and signed.
** 2 testimonials of character and conduct by Gazetted Officers / Members of Parliament / Member of Legislative Assembly / Magistrate Ist Class and the principals and professors of Medical Colleges.
** Permanent Registration Certificate.
** Additional Registration Certificate.
** MBBS Degree
** P.G. Degree
** Passing Certificate of P.G. Diploma / Degree issued by the Dean of concerned Medical College.
** A Bank Draft of Rs.2000.00 in favor of the “The Secretary, Medical Council Of India, New Delhi” (Payable at New Delhi) (Postage charges extra, if applicable).
** A Bank Draft of Rs.2500.00 in favor of the “The Registrar, Madhya Pradesh Medical Council: Bhopal” as Forwarding fee. (see fee chart).

REQUIREMENTS FOR OBTAINING N. O. C. ABOUT REGISTRATION :
** An Application on plain paper addressing to the Registrar, M. P. Medical Council.
** Photocopy of Registration Certificate

REQUIREMENTS FOR CHANGE IN SURNAME :(ONLY FEMALE CASE)
** An Application on plain paper addressing to the Registrar, M. P. Medical Council.
** An Affidavit or Marriage Certificate
** Original Registration Certificate (Without lamination)
** The prescribed fee (see fee chart).

REQUIREMENTS FOR CHANGE IN ADDRESS :

Functions of MPMC

The Madhya Pradesh Medical Council (MPMC) is a statutory body constituted under the Madhya Pradesh Ayurvigyan Parishad Adhiniyam, 1987, to regulate the allopathic medical profession in Madhya Pradesh.

The MPMC has the following functions:
** To register all medical practitioners in Madhya Pradesh.
** To maintain a register of all registered medical practitioners.
** To grant provisional registration to medical practitioners who have completed their internship but have not yet passed the final MBBS examination.
** To grant permanent registration to medical practitioners who have passed the final MBBS examination and have completed their internship.
** To issue certificates of registration to registered medical practitioners.
** To conduct disciplinary proceedings against registered medical practitioners who have violated the Medical Council of India (MCI) Code of Ethics.
** To suspend or cancel the registration of registered medical practitioners who have been found guilty of misconduct.
** To promote the study and practice of medicine in Madhya Pradesh.
** To provide for the education and training of medical practitioners in Madhya Pradesh.
** To advise the government on matters relating to the medical profession in Madhya Pradesh.

  1. I have done renewal of my MP state pharmacy council certificate its showing certificate generated and digital sign done.

    but not able to download
